The Council recognises the need to communicate clearly and more regularly with people, so that they know what activities are available, and when.
Currently, following a Care Act assessment, the Council will share information about day services available to people within the local area or direct people to the Live Well Cheshire West website for information about day services providers. Live Well Cheshire West is a website providing details of how to contact organisations that deliver a range of services, including day services.
To improve the way we share information about the activities available, we propose to:
send out a monthly ‘What’s on’ email to everyone signed up to a mailing list
list all services available on the Council’s Live Well Cheshire West website
provide a catalogue and location map of all the different activities. This would be distributed by a social worker and be available at key locations where services are provided in the future.
We have provided an example to help explain what this idea would mean to people using day services. This example is not based on a real person, it is only provided to help illustrate this idea.
What happens currently for people who use our services
Emma can access information on the day services available through her social worker or the Live Well Cheshire West website. This is then arranged on her behalf by the Council.
What this would look like in the future for people who use our services
